Online Course Permission Request Form - Non-Credit Classes Summer 2008

To obtain permission to register for the non-credit online courses listed below, please complete the following questionnaire.

You will be asked to give some basic information about yourself (name, e-mail address, etc) and to respond to some questions. These questions are designed to help you understand what to expect from an online course and to prepare you for online learning.

After you complete and submit the online questionnaire, your answers will be forwarded to the instructor or program director. You will receive an e-mail reply with very important information about how to register for and begin your online course.

You cannot register for an online course until you have completed this online form and received an e-mail reply with permission to register.
